In the Pacific, the first Tahitian death on the ship Bounty (of the famous Mutiny on the Bounty) was the ship's surgeon after a fish feast: "Old Bacchus died not of drink, as might have been supposed, but of eating a poisonous fish" - "Mutiny on the Bounty," Nordoff and Hall 1932.
Most predatory reef fish (which is most of them) are known to carry ciguatera (a marine toxin) that builds up to possibly lethal levels in the larger fish. Whenever you are fishing in tropical areas you should enquire about local fish associated with Ciguatera. If you follow the local guidelines this will not become a problem.
